We have been advised by the PHU that a COVID booster vaccination clinic will be held on-site the 19th and 20th of January.
If you wish for the person that you care for to have their booster and you have not already done so can you please complete the consent form and return it to us.
If you do not wish for them to receive the booster and you have not already done so please email us to advise accordingly so we can note this on file.
